To enroll your child at The Toddler House Academy your child must be between the ages of 6 weeks- 36 months

The following forms must be completed to reserve your child’s spot at The Toddler House Academy.

- Money Order/ Zelle/ PayPal for Registration & Supply Fee
- Child’s Medical Information and Form
- Child’s Birth Certificate
- Parent Contact Information Form
- Proof of Immunizations
